Clinic Hours
Revive MD Internal Medicine  is open from 8:30 a.m. to 5 p.m.  The office is closed on Saturday and Sunday and all major holidays.

Appointments
Appointments to see Dr Gorthi can be made by calling (321) 425-3187.

Please give at least 24 hours notice if you must cancel or reschedule your appointment. This will allow us to schedule another patient that may need to see the physician sooner than their scheduled appointment.  Please see our office and financial policy for "no shows" and less than 24 hr cancellations.

Please bring all of your medications and your insurance information with you each time you come for a physician appointment. 


Lab and Diagnostic Tests
It is best that lab work or any diagnostic studies be performed at least 3-7 days prior to your physician appointment. This will ensure that the results will be ready for your physician to review with you at the time of your appointment.  If you have not received your test results within five business days of the date performed, please call the clinic and ask to speak with your nurse.

Telephone Calls
Dialing (321) 425-3187 will connect you with the Revive MD Internal Medicine switchboard. Your call will be screened so that your message can reach its appropriate destination.

If you need to speak to the doctor, a message will be taken. The message and your medical record will be delivered to your doctor and your call will be returned ASAP with the recommendations of the doctor. Please be aware that it may be longer depending on the availability of your physician.

If you need to speak to the business office regarding an insurance issue or status of your account, you may call our billing office at 321-795-6380.

If you have an emergency situation, you should go to the nearest emergency room.
If you need to be seen or evaluated after hours or on holidays please go to your nearest urgent care and notify us the next business day for continuity of care.
All non emergent calls and voicemails after hours and holidays will be returned ASAP the next business day.  Dr. Gorthi will only be paged for emergencies. 

Prescription Refills
  • It is your responsibility to notify the office in a timely manner when refills are necessary. Approval of your refill may take up to three business days so please be courteous and do not wait to call. If you use a mail order pharmacy, please contact us fourteen (14) days before your medication is due to run out.
  • Medication refills will only be addressed during regular office hours (Monday-Friday 8am-5pm). Urgent phone calls will not be addressed regarding refills. Please notify your provider on the next business day if you find yourself out of medication after hours. No prescriptions will be refilled on Saturday, Sunday or Holidays.
  • Refills can only be authorized on medication prescribed by providers from our office. We will not refill medications prescribed by other providers.
  • Some medications require prior authorization. Depending on your insurance this process may involve several steps by both your pharmacy and your provider. The providers and pharmacies are familiar with this process and will handle the prior authorization as quickly as possible. Only your pharmacy is notified of the approval status. Neither the pharmacy nor the provider can guaranty that your insurance company will approve the medication. Please check with your pharmacy or your insurance company for updates.
  • It is important to keep your scheduled appointment to ensure that you receive timely refills. Repeated no shows or cancellations will result in a denial of refills. All prescriptions require a follow up appointment every 3 to 6 months.
  • If you have any questions regarding medications please discuss these during your appointment. If for any reason you feel your medication needs to be adjusted or changed please contact us immediately.
  • New symptoms or events require a clinic appointment. Your provider will not diagnose or treat over the phone.
